Probate Forms (AK)
There are a number of state-specific forms you may need when going through the probate process.
General AK Forms
In Alaska, estate probate is handled by a Superior Court in the judicial district in which the decedent lived.
You can check with your local court for the forms they might prefer, and below you will find some of the more common AK forms:
- Request for Informal Probate (Form P-315) - Get the probate process started if there is a will
- Statement Starting Informal Probate (Form P-315) - Include with request there is a will
- Acceptance of Duties by Personal Representative (Form P-335) - Include with request if there is a will
- Request for Informal Probate without Will (Form P-325) - Get the probate process started if there is no will
- Statement Starting Informal Probate without Will (Form P-326) - Include with request there is no will
- Acceptance of Duties by Personal Representative with No Will (Form P-336) - Attach to request if there is no will
- Nomination for Appointment of Personal Representative with Lower Priority (Form P-306) - Include with your request if others outrank you
- Waiver of Bond Requirement (Form P-334) - Attach to request if desired and not addressed by will
- Notify Heirs (Form P-340) - Notify heirs and devisees about the probate
- Notify Creditors (Form P-341) - Notify creditors about the death
- Affidavit of Publication (Form P-342) - Tell court you published creditor notice
- Inventory (Form P-370) - Tell court about estate contents (consider attaching EstateExec Inventory Report)
- Final Accounting and Proposed Distribution (Form P-380) - Request court's permission to distribute estate
- Receipt and Release (Form P-385) - Get receipts from distribution recipients
- Closing Statement (Form P-355) - Tell the court you are done
