Probate Forms (KY)

There are a number of state-specific forms you may need when going through the probate process.

General KY Forms

In Kentucky, the local District Court handles wills and estate probate. To find your court, select Category "District Judges", and then the county of the decedent's legal residence.

You can check with your local court for the forms they might prefer, and below you will find some of the more common KY forms:

  1. Petition for Probate (Form AOC-805) - Get the probate process started
  2. Fiduciary Bond (Form AOC-825) - Tell the court you have obtained any required bond
  3. Inventory and Appraisement of Estate (Form AOC-841) - Consider attaching EstateExec Inventory Report
  4. Informal Settlement of Estate (Form AOC-850) - Request simple completion of estate
  5. Standard Settlement of Estate (Form AOC-846) - Consider attaching EstateExec Final Accounting Report
